It sounds like your chain, and sprockets are worn. Try loosening the chain to 1 1/2 inch play. See if it makes a difference.

The chain hasn't stretched much (I still have a lot of adjustment left) & ter rear sprocket looks good. The chain may need to be soaked & lubed.

I'l loosen her up to see if it improves since I only noticed the issue AFTER I adjusted it.
